I like how throwing knifes and spears will be tied to the related melee weapon skill instead of a generic throwing skill. But im not convinced on having grenades related to melee.
As grenades are more a situational weapons, one approach can be to use a non skill related formula (let say 55%...
I think some of the 2-handed ST requirements are too low, like Sniper/assault rifle, shotguns, and 223 pistol.

I really like the approach you are taking on making the game more realistic and balanced.
I also like the changes on traits/perks, specially the proposed perks that change the behavior of NPC in combat, as they can add new and fresh gameplay.
