Okay, so hear me out. It's not perfect. I got the sheet from this website and desired to make this a reality.
I initially wanted to make a Spreadsheet version that would auto-fill fields and be awesome.
But I have no idea how to actually do all that, so using my basic know-how I made this instead.
Feel free to use it all you want. When you open it, you have to click File in the top left, then you can save your own copy.

It uses Google Drive, so if you don't use that sorry.
Drive is free and super useful, maybe consider beginning to use it if you don't.
