I've played this game a few times and only infected Madagascar like once.
Last game I played, got every country but Madagascar (Fucking isolationist pricks.) Protip: I've found being in a lot of countries or being lethal triggers the vaccine to be made, so once you get into every country, ramp up your lethality. Your disease should still spread and with any luck, kill enough to shut down hospitals.
Unless the hospital does what Greenland did, to their last 5 or so people they worked on developing and distributing the cure. It didn't work (and I think everyone was infected anyway) and they died.
